Statistical methods
Survival rates were calculated by the Kaplan–Meier method and compared using the log rank test; survival graphs were truncated when the number of recipients remaining was ≤10. Multivariate analyses were performed by logistic regression, and weights were used to account for incomplete follow-up. Centers and Activity
The number of centers reporting lung and heart–lung transplantations each year is shown in Figure 1. Although the number of centers has been nearly stable since 1997, annual activity increased by an average of 3.6% from 1997 to 2004, and volume reached an all-time high of 1,815 transplants in 2004 (Figure 2). The number of single-lung transplantations performed each year has been almost constant for the last decade, but the number of bilateral transplantations has more than doubled since 1994.
Centers and Activity
The number of centers reporting heart–lung transplantations has decreased from a maximum of 63 in 1994 to 37 in 2004 (Figure 1). Annual activity peaked in 1989 and, with the exception of a brief surge in 1995 and 1996, declined thereafter. Since 2001, activity has stabilized in the range of 76 to 91 procedures per year (Figure 25). Only five centers reported average activity of ≥4 heart–lung transplantations per year for the period January 1998 through June 2005, and 68% of the transplantations
