Recipient Characteristics
Primary diagnoses for all donors are reported in Table 1. Patients receiving HSD lungs were less likely to have a primary diagnosis of idiopathic pulmonary fibrosis (21.8 vs 25.6%, p = 0.027) and more likely to have emphysema/chronic obstructive pulmonary disease (30.3% vs 23.8%, p = 0.001). Recipient characteristics are reported in Table 2. Recipients of HSD lungs had longer ischemic time (5.75 vs 5.58 hours, p = 0.012), lower percent predicted FEV1 pretransplantation (34.3% vs 36.1%, p =
Comment
Currently accepted criterion for donor smoking history in lung transplantation is less than 20 pack-years [5]. However, a shortage of organ donors has led to increased efforts in expanding the donor pool 3, 4, 5, 6, 7, 12, 13. A donor with a smoking history of longer than 20 pack-years falls into the category of a marginal donor [5]. Although several studies have shown that transplantation using marginal donors can be safe 2, 6, 8, 9, 10, 11, 14, none of these studies has sufficient numbers for

In the case of smoking history, studies assessing long-term survival find that the risk of donor smoking history is not significant at later time points.12,13 Interestingly, rate of utilization of lungs with significant smoking history declined over the study period.
