Zusammenfassung
Vorgestellt wird eine 52-jährige Frau, die nach einem Marathonlauf eine akute hyponatriämische Enzephalopathie entwickelt, gefolgt von einem beatmungspflichtigen, nichtkardiogenen Lungenödem. Die anstrengungsassoziierte Hyponatriämie bei Ausdauersportlern ist eine lebensbedrohliche Komplikation. Eine übermäßige Zufuhr von freiem Wasser während oder nach der Belastung bei persistierender Antidiurese infolge anhaltender ADH-Freisetzung durch nichtosmotische Stimuli gilt als Hauptursache. Als Risikofaktoren gelten u. a. weibliches Geschlecht, lange Laufzeit und fehlender Gewichtsverlust. Die Notfalltherapie besteht in einer Flüssigkeitsrestriktion und der Bolusgabe von hypertoner 3%iger NaCl-Lösung zur schnellen Senkung des Hirndrucks.
Abstract
This article presents the case of a 52-year-old woman who developed exercise-associated hyponatremia (EAH) complicated by non-cardiogenic pulmonary edema after a marathon run. The condition of EAH is a potentially life-threatening complication of endurance exercise. The main cause seems to be inadequate intake of free water during or following exercise with enduring antidiuresis due to nonosmotic stimulation of ADH secretion. Known risk factors are female gender, slow running pace and lack of weight loss. Emergency therapy is fluid restriction and bolus infusion of 3 % NaCl solution to rapidly reduce brain edema.
