Abstract

In 186 patients with Down's syndrome (age 1–68 yr) total cholesterol and triglycerides were determined, quantitative lipoprotein electrophoresis was performed and cholesterol fractions and β/a-lipoprotein ratios were calculated. A reference group of 51 non-mongoloid mentally handicapped adults living in the same institutions were examined in the same way. Patients' total cholesterol, β-cholesterol and triglycerides did not differ from the controls. However, αcholesterol was significantly lower and the β/α-lipoprotein ratio significantly higher in patients, findings which are associated in the general population with a high risk for premature atherosclerosis. Interestingly, mortality causes and pathological findings in Down's syndrome show no increased frequency of cardiovascular diseases.

Moreover, healthy older individuals with DS show some classic biochemical risk factors for atherosclerosis but do not suffer from clinical cardiovascular alterations [9]. Individuals with DS present several metabolic lipid abnormalities [7,10–15], although serum lipoprotein profiles cannot completely explain the lower prevalence of cardiovascular disease. Studies have shown that younger individuals with DS have lower high-density lipoprotein (HDL) levels [11,13,14], altered lecithin:cholesterol acyltransferase (LCAT) activity [16] and HDL component apolipoprotein A-I (ApoA-I) levels [12].

In patients with Down syndrome, prevalence of overweight and obesity is significantly higher, compared to other patients with intellectual disabilities and to the general population, with a reported prevalence of obesity in adults with Down syndrome between 31 and 47% (Bell & Bhate, 1992; Melville, Cooper, McGrother, Thorp, & Collacott, 2005; Prasher, 1995). Although abnormalities in lipid metabolism, which are associated with high risk of premature atherosclerosis in the general population, are frequently seen in patients with Down syndrome (Dorner, Gaethke, Tolksdorf, Schumann, & Gustmann, 1984), coronary artery disease-related mortality is surprisingly low (Baird & Sadovnick, 1988). Histopathological investigations showed no increase in atherosclerosis, or even a total lack of atherosclerotic changes.

Apolipoprotein A1 levels were significantly decreased in AFS of both trisomy 18 and trisomy 21. Although decreased serum levels of apolipoproteins A1 have been frequently found in individuals with DS (18, 25–27), its decreased levels in trisomy 18 in our supernatant analysis might be partly because the amniotic fluid contained a myriad of fetal cells from various origins, or it was a maternal response to aneuploidy fetuses given the proximity of decidua to fetal membranes. Adaptor-related protein complex 3μ 2 subunit (AP3M2, AP-3μ) is a subunit of the heterotetrameric adaptor-related protein complex 3 (AP-3) that belongs to the adaptor complexes medium subunits family.
